What we do — and how we do it
Connected Futures delivers four service streams across the North East of England, all underpinned by the same commitment to quality housing management, professional compliance and genuine partnership with commissioners and referral agencies.
Service streams
The pathway below moves from emergency provision through to supported accommodation and on to transitional housing — reflecting the journey we aim to support people along, from crisis toward confidence and independence.
Temporary Accommodation
Emergency and short-term housing, commissioned directly by Local Authorities.
- For individuals and families in housing crisis who need immediate, safe accommodation
- Commissioned directly by Local Authorities or on a nightly or block basis
- All properties managed to the same standards as our wider portfolio — compliant, well-maintained and with responsive management in place
- Suitable for: homeless individuals and families, emergency placements, individuals leaving institutional settings
Supported Accommodation
For individuals with complex or multiple support needs — delivered through our Registered Provider partnerships.
- Intensive Housing Management delivered in partnership with our Registered Providers
- Dedicated Housing Support Officers working individually with each resident
- Individual support plans developed with and for each person
- For adults with mental health needs, substance misuse histories, domestic abuse experience, offending backgrounds, learning disabilities and other complex needs
Transitional Housing
The UC+ model — for people already well along the journey toward independent living.
- For individuals who have achieved a degree of stability and are ready for the next step
- Funded through the Universal Credit housing element, paid direct by DWP
- Delivered in partnership with our Registered Providers
- Tenancy Sustainment Officers providing regular visits and practical support
- Benefits casework, move-on planning and community connection
- The positive destination on the housing pathway — managed well to help people hold on to what they have built
Ascent
Connected Futures' young people's service. Supporting care leavers, care experienced young people and unaccompanied young people to build stability, confidence and independent futures.
- Specialist accommodation and support for care leavers and care experienced young people
- Currently providing for young people aged 18 and over — actively developing our pathway for 16 and 17-year-olds
- Working in close partnership with Local Authority leaving care and children's services teams
- Intensive support focused on building life skills, confidence and the foundations for adult independence
- Developed in direct response to commissioned need across the North East — actively expanding

Housing management across all streams
Regardless of the funding model or service stream, every Connected Futures property is managed to the same standard:
- Regular property inspections and responsive maintenance
- Fire safety compliance and annual certification
- Gas and electrical certification
- Comprehensive tenancy documentation
- Safeguarding-aware management throughout
- Full compliance with all applicable regulatory requirements
